Lead Capture Bad server response

When I try to create an entity it will not save the entity, and pops up with a box saying bad server response.
Please guide me

Ho to delete failed log
Administration > Scheduled Job > Check Personal Email Accounts. Make sure there are no records with failed status.

Now I able to create Lead capture but when I try to post data using form I got an error
