Though more often they are skipped intentionally. For example, having Follow icon but not having Unfollow in mass actions is quite reasonable. Even if we had an appropriate icon I'd prefer not including it there.

Actually both. In some cases there's no icon because the action has low importance. In some cases, we just don't have an appropriate icon, or one that we have stands out too much (different style) drawing attention to itself from more important actions.

i understand - are the other actions messing icons because there are not any available icons that would fit them in the current used set of icons ? or was that a design decision made?

    Just curious to know

It was fine in windows 98 not having all icons. MacOS did the same too up until recently.

While it may feel heavier it serves accessibility purpose. They really do their job well.

    started a topic Menu icons

    Menu icons

After using the v10 for little. I think adding icons to menu actions feels a little heavy and even some actions have icons some don't which makes it looks like issue with displaying icons e.g below

Save & Continue Editing - has icon
    Save & New - doesn't have icon

same for mass actions as per image attached

i always preferred the version without icons - simple and actions name were clearly easy to understand.
