We have another app that we use where customers can generate a one time use code for licensing, It would be really cool if you could use this to submit a form, and be able to setup the response to create an record from the response.

You can try to implement such logic by using the manual Workflow rule with a button and Send HHTP Request action:

As a result, this button will be able to trigger a workflow rule with the action Send HHTP Request to send data to your site.
