Update User's Roles

I can't get a user's permissions to update, if I create a user then assign a role afterwards it will not change the permission even after clearing the cache and logging out and back in. Same with users whose roles I edit. I can see the changes I've made to the roles but the user's permission remain the same. I also tried removing their role and then applying it again per user and with mass update neither method works. Any suggestions?
