after Upgrade from 3.0.1 to 3.1 i get "500 - Internal Server Error"

after i have install the patch and the upgrade from 3.01 to 3.1 i get "500 - Internal Server Error".

in the log file (in the cpanel)
i get this error:
[2015-03-20 13:41:05]: error: file is writable by others: (/home/public_html/crm/index.php)

ok i now can access to admin. the admin in not showing any data and i get this errors:

Check if there is duplicate entries in entity_team table. And remove them.
