Add entire enum list to form

I would like to add an entire list to a form that gets printed as a traveler document with an item of equipment. The user will select one of the issues, circle it and fill the rest of the form in. On return the selected issue will be entered into the CRM system. I am making a printable form using templates and wondering if the form can automatically get the enum values and add to the form instead of the selected value?
