Add new fields to create contact

I am using ESPO Cloud. When I want to create a new contact, I need a column for middle name. I have already created the VARCHAR field, but how do I add it to this form? Guidance appreciated.

You can add the field via Layout Manager under Administrator. If you want to add the middle name in the same line, then need code modification.
