Problem with vanishing attachments

I have a problem from time to time with attachments (images) vanishing as well as from the data-record (upload folder) as from the database. All this has no reckognizable pattern and occurs suddenly.
It does happen without anything had been changed for months in the data-record. I have currently one record, which was created in march 2023 and one attachment (of three) vanished in may 2024, when I substituted it. And now it is gone again.
I saw, that this problem had been discussed before, but does not seem to be solved.

Do u have custom script on an image field? like a script make the same attachment shared between several records?

Yes I have an entity, which uses images from another entity, by formula and relationship.
