A way to convert data from entity to another

Hello
i create a new entity which name forms
that includes the same contacts data
but its designed for a specific purpose
All of our data records as a forms
then some one contact them to make a contract
to be contacts
so i want a way to make a buton on forms
can convert it to contact directly
now
forms recorded , staff call them
then re-record form as a contact
