Life Insurance Quoter - Function on Lead Screen

I'm looking to have a life insurance quoter built into the lead screen. In essence, when speaking with a client I want the ability to pull certain info from the lead screen such as postal code, age, and a couple of additional lead fields to generate a life insurance quote. I can provide some mock screens to further define the project. I would also need the ability to add or remove insurance carriers from time to time as well as email the quote to the lead.

Your post sound like you want to hire a developer rather than asking for support with getting certain feature and functions to work (which is what this forum section I think).

Or you just want suggestion on how to implement this plan of yours?
