Unable to create new organization

Hi, I'm currently new into CRM. Right now I am installing the CRM splitted into 3 server. 1 for APP, 1 for ASYNC, and 1 for DB. Right now I am able to open my Deployment Manager but I can't create new organization. Can anyone help me with this? I attach notepad of the error here. I need the help urgent regarding to this. Thank you very much!
