Adding href to links in stream notes

Which script is responsible for automatic <href> tag add to all the links posted on entity comments/description etc. I need to add _blank attribute to it, but there is numerous scripts which does this thing throuought the system.

You can create URL field in Entity Manager, make it readOnly and set the value in Entity Formula
